Tesamorelin is a synthetic analogue of human growth hormone-releasing hormone, GHRH(1-44), modified by a trans-3-hexenoyl group at the N-terminus. That acylation is the defining structural feature: native GHRH is cleaved rapidly by dipeptidyl peptidase-4 at the Ala2 position, and the hexenoyl modification substantially slows that degradation, extending the half-life of the peptide relative to the unmodified sequence. It acts as an agonist at the GHRH receptor on somatotroph cells of the anterior pituitary, stimulating endogenous growth hormone release in a pulsatile pattern rather than supplying exogenous hormone directly.
